of the Immigration process simple and transparent.
Our approach thus is clear and lays out all involved costs with no hidden charges.
To make the process simple, we have an on-site lab at the office for blood and urine tests
What to Bring to the appointment?
§ Form I-693 (https://www.uscis.gov/sites/default/files/document/forms/i- 693.pdf)
§ In the Form I-693, fill out “Part 1. Information About You”. Do not sign the form until the civil surgeon tells you to sign it. You must sign in the presence of the civil surgeon.
§ Government-issued photo identification, such as a valid passport or driver’s license.
§ Report of previous Medical Examination if available.
§ Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease) if available.
§ Payment (Credit card or Cash are both acceptable at our location).

THE EXAM
Immigration Physical Exam and Mental Status Evaluation by the Civil Surgeon are conducted at our office.Same day appointments are available by calling our office at (281) 240-0311

VACCINATIONS

Returning to the Office

As a final step, you will need to return to the office in approximately 7-10 days. This time period will allow for test results to come back and vaccinations to be taken. During this visit, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS and provide you a copy for your personal records. Please note that USCIS will not accept the form if the envelope has been opened or altered.

§ Immigration Physical Exam (covers the exam and cost of filling up form I-693)
§ TB testing (IGRA blood test)
§ Syphilis testing (ages 15 and above)
§ N. Gonorrhea testing (ages 15 and above)
§ Varicella immunity (chicken pox) titers (if you have had a history of chicken pox)
§ Urine Drug Screening (only if required)
IMPORTANT CONSIDERATIONS

What if Quantiferon is Positive?
Could I need other treatment?
Yes. If the other tests for Syphilis or Gonorrhea are positive, you may be referred for treatment before the Immigration paperwork can be advanced.
