Fragmentation of care and lack of continuity when patients transition from the hospital to a post-acute care facility are the most important reasons for sub-optimal outcomes for patients and re-admission to the hospital. With that in mind, the goal of the Post Discharge Patient Management Services program is to make the transition seamless and avoid lapses in transfer of critical information such as critical medications, follow-ups and important labs. Our highly qualified team led by Board Certified Physicians Dr. Ranganath Kandala, Dr. Vijay Korimilli and their Nurse Practitioners will ensure that we care for your loved ones like we would care for our own.
